2. MEDICAL PROTOCOL FROM THE LKK RELATED TO THE ILLNESS.

3. WITHIN 7 DAYS OF THE PROTOCOL'S ISSUANCE, YOU WILL RECEIVE APPROVAL FROM THE REGIONAL HEALTH INSURANCE FUND (RZOK) - NUMBER AND DATE - WHICH WILL BE COMMUNICATED TO YOU BY PHONE OR VIA AN EMAIL ADDRESS PROVIDED BY YOU.

4. YOU INFORM US OF THE APPROVAL RECEIVED FROM THE RZOK AND RECEIVE YOUR APPROVED FOREARM CRUTCH.

5. IT IS GRANTED FOR 3 YEARS, AFTER WHICH YOU CAN REAPPLY FOLLOWING THE SAME PROCEDURE.
