3. WITHIN 7 DAYS AFTER THE PROTOCOL IS ISSUED, YOU WILL RECEIVE APPROVAL FROM THE RHIF - NUMBER AND DATE - YOU WILL BE NOTIFIED BY PHONE OR TO AN EMAIL ADDRESS PROVIDED BY YOU.

4. YOU INFORM US OF THE RECEIVED APPROVAL FROM THE RHIF AND RECEIVE YOUR ALLOCATED CANE.

5. IT IS ALLOCATED FOR 3 YEARS, AFTER WHICH YOU CAN RE-APPLY FOLLOWING THE SAME PROCEDURE.
