1. IF YOU ARE NOT APPLYING IN PERSON, BUT THROUGH ANOTHER PERSON, A POWER OF ATTORNEY IS REQUIRED FOR THE MEDICAL EXPERT COMMISSION (LKK) AND THE NATIONAL HEALTH INSURANCE FUND (NZOK). THE POWER OF ATTORNEY DOES NOT NEED TO BE NOTARIZED.

2. MEDICAL PROTOCOL FROM THE LKK REGARDING THE SPECIFIC MEDICAL CONDITION.

3. WITHIN 7 DAYS OF THE PROTOCOL'S ISSUANCE, YOU WILL RECEIVE APPROVAL FROM THE REGIONAL HEALTH INSURANCE FUND (RZOK) - A NUMBER AND DATE - WHICH WILL BE COMMUNICATED TO YOU BY PHONE OR TO AN EMAIL ADDRESS YOU PROVIDE.

4. YOU INFORM US OF THE APPROVAL RECEIVED FROM THE RZOK AND RECEIVE YOUR ALLOCATED FOREARM CRUTCH.

5. IT IS ISSUED FOR 3 YEARS, AFTER WHICH YOU MAY REAPPLY FOLLOWING THE SAME PROCEDURE.
