2. MEDICAL PROTOCOL FROM THE LKK REGARDING THE DISEASE PROFILE.

3. WITHIN 7 DAYS OF THE PROTOCOL'S ISSUANCE, YOU WILL RECEIVE APPROVAL FROM THE REGIONAL HEALTH INSURANCE FUND (RCIZ) - A NUMBER AND A DATE - WHICH WILL BE COMMUNICATED TO YOU BY PHONE OR TO AN EMAIL ADDRESS PROVIDED BY YOU.

4. YOU NOTIFY US OF THE RECEIVED APPROVAL FROM THE RCIZ AND RECEIVE YOUR ALLOTTED CANE.

5. IT IS ISSUED FOR 3 YEARS, AFTER WHICH YOU CAN REAPPLY FOLLOWING THE SAME PROCEDURE.
