2. MEDICAL PROTOCOL FROM THE LKK REGARDING THE DISEASE PROFILE.

3. WITHIN 7 DAYS OF THE PROTOCOL'S ISSUANCE, YOU WILL RECEIVE APPROVAL FROM THE RHIF - A NUMBER AND DATE - WHICH WILL BE COMMUNICATED TO YOU BY PHONE OR TO THE EMAIL ADDRESS YOU PROVIDED.

4. YOU INFORM US OF THE RECEIVED APPROVAL FROM THE RHIF AND RECEIVE THE WALKER THAT HAS BEEN GRANTED TO YOU.

5. IT IS GRANTED FOR 3 YEARS, AFTER WHICH YOU CAN RE-APPLY FOLLOWING THE SAME PROCEDURE.
