2. MEDICAL PROTOCOL FROM LKC ACCORDING TO THE DISEASE PROFILE.

3. WITHIN 7 DAYS OF THE PROTOCOL'S ISSUANCE, YOU WILL RECEIVE APPROVAL FROM THE RHIF - NUMBER AND DATE - WHICH WILL BE COMMUNICATED TO YOU BY PHONE OR TO AN EMAIL ADDRESS YOU PROVIDE.

4. YOU NOTIFY US OF THE RECEIVED RHIF APPROVAL AND RECEIVE YOUR ALLOCATED WALKER.

5. IT IS ISSUED FOR 3 YEARS, AFTER WHICH YOU CAN APPLY AGAIN FOLLOWING THE SAME PROCEDURE.
