1. IF YOU ARE NOT APPLYING PERSONALLY, BUT THROUGH ANOTHER PERSON, A POWER OF ATTORNEY IS REQUIRED FOR THE MEDICAL EXPERT COMMISSION (LKK) AND THE NATIONAL HEALTH INSURANCE FUND (NHIF). THE POWER OF ATTORNEY DOES NOT NEED TO BE NOTARIZED.

2. ELECTRONIC MEDICAL PROTOCOL ISSUED BY THE MEDICAL EXPERT COMMISSION (LKK) ACCORDING TO THE PROFILE OF THE DISEASE - NEUROLOGICAL, ORTHOPEDIC, SURGICAL OR ONCOLOGICAL

3. WITHIN 7 DAYS OF THE PROTOCOL'S ISSUANCE, YOU WILL RECEIVE APPROVAL FROM THE REGIONAL HEALTH INSURANCE FUND (RHIF) - A NUMBER AND DATE - WHICH WILL BE COMMUNICATED TO YOU BY PHONE VIA VIBER / SMS.

4. YOU INFORM US OF THE RECEIVED RHIF APPROVAL AND RECEIVE THE ALLOCATED COMBINED TOILET CHAIR.

5. THEY ARE GRANTED FOR 6 YEARS, AFTER WHICH YOU CAN APPLY AGAIN THROUGH THE SAME PROCEDURE.

6. WITHIN THE OPERATIONAL PERIOD, YOU ARE ENTITLED TO TARGETED AID FOR REPAIRS, TIRES, AND BEARINGS.
