2. MEDICAL PROTOCOL FROM THE LKK REGARDING THE DIAGNOSIS OF THE ILLNESS.

3. WITHIN 7 DAYS OF THE PROTOCOL'S ISSUANCE, YOU WILL RECEIVE APPROVAL FROM THE RHIF (REGIONAL HEALTH INSURANCE FUND) - A NUMBER AND DATE - WHICH WILL BE COMMUNICATED TO YOU BY PHONE OR VIA THE EMAIL ADDRESS YOU PROVIDE.

4. YOU NOTIFY US OF THE APPROVAL RECEIVED FROM THE RHIF AND RECEIVE YOUR ALLOCATED ANTI-DECUBITUS PILLOW.

5. IT IS ISSUED FOR 3 YEARS, AFTER WHICH YOU CAN RE-APPLY FOLLOWING THE SAME PROCEDURE.
